Becoming an ASDS Member

The steps below explain the membership application process, and contain links to forms and information.

• Fill out a  membership application  or apply online 
• Submit 3 letters of endorsement from ASDS Fellows*
           *AAD members and Corresponding Fellow applicants are only required to get one letter
• Application fee of $95.00 may be paid by credit card during online payment, or sent via email to hprostko@asds.net or fax to 847-956-0999. Check payments and documents may be mailed to:
